Expanded Medi-Cal Coverage in 2025 for Undocumented Immigrants in California
Assembly Bill AB-4 marks a significant step forward in healthcare access for undocumented individuals in California.
While undocumented immigrants can already access Medi-Cal, they are still barred from enrolling in Covered California.
The new bill seeks to change this and include them in the state insurance marketplace.
What is Medi-Cal?

Medi-Cal is California’s state health insurance program, jointly funded by the state and federal governments.
It provides free or low-cost medical coverage to those who meet certain residency and income criteria.
It is one of the main tools the state uses to ensure access to essential health services, such as:
- Medical care;
- Prescription medications;
- Hospitalization; and
- Other healthcare services.
Key Points of the Medi-Cal Proposal for Undocumented Immigrants
Some highlights of the proposal include:
- It would allow undocumented individuals to enroll in Covered California.
- The bill does not require additional federal authorization, which would simplify its implementation.
- The proposal comes after the state requested a $3.4 billion loan due to the growing number of Medi-Cal beneficiaries.
- It would benefit those who exceed the income threshold for Medi-Cal but cannot qualify for private insurance due to their immigration status. In some cases, they could explore different types of immigration waivers to regularize their status and access broader benefits.
Challenges to the Medi-Cal Bill for Undocumented Immigrants
Although the new bill aims to expand healthcare access for undocumented immigrants, it faces several challenges:
- The state is spending millions more than projected to cover undocumented individuals, raising concerns about sustainability.
- Governor Newsom’s administration requested additional funds but has not clearly stated when they will be restored.
- Republican legislators have questioned the spending on undocumented coverage and demanded more transparency.
- In addition to spending on the migrant population, the program also faces increased costs due to other difficulties.
- Proposals in Congress could reduce the national Medicaid budget, directly affecting state funding for Medi-Cal.
Despite these obstacles, Democrats have reaffirmed their commitment to expanding coverage.
Who Qualifies for Free Medi-Cal in California?
Most immigrants in California may qualify for health coverage through Medi-Cal or a Covered California plan.
This includes both immigrants with legal status and those without, as long as they meet eligibility criteria.
Qualifying groups include:
- Lawful permanent residents (Green Card holders);
- Lawful temporary residents;
- Beneficiaries of humanitarian programs like TPS in the US.;
- DACA recipients (from November 2024, if they meet certain criteria);
- Non-immigrant visa holders;
- Refugees, asylees, and others fleeing persecution.
What Do New Enrollees Say About Medi-Cal?
A study of nearly 100 Latino, Chinese, and Korean immigrants revealed key insights:
- 70% enrolled successfully, especially those previously using restricted-scope Medi-Cal;
- 88% used medical services, mainly for primary, preventive, and dental care;
- High satisfaction with the care received;
- Little political concern, with most feeling protected in California and continuing to use the program unless laws change.
Still, the study warns that these results may not reflect the broader undocumented population.
What Does Medi-Cal Offer and Who Can Qualify?
Applying for or using Medi-Cal does not affect immigration status. Anyone who meets the requirements can apply.
Requirements include:
- Living in California;
- Meeting income and household size limits.
What Medical Services Are Covered by Medi-Cal?

Covered services include:
- Dental and vision care (including eyeglasses);
- Hearing aids and medical transportation;
- Referrals to specialists;
- Prescription drugs;
- Mental health and substance use treatment;
- Emergency services, in-home and long-term care;
- Vaccines;
- Medical visits.
Note: All services are free for eligible individuals.
